There are a number of fillers available on the market. Some fillers made of hyaluronic acid, a natural substance found in all living organisms. Other fillers are made of various other compounds, some natural, and synthetic products. The type of filler that is used determines the safety, efficacy as well as the beauty of the final product of your treatment.

By adding the fillers, the skin’s natural hyaluronic acid levels and contours of the skin are restored, re-creating that youthful appearance.

As a result of aging, our cells gradually lose the ability to produce hyaluronic acid, collagen, fat, bone and other important ingredients in the rejuvenation process of the face! Therefore, we need to use medical grade products as fillers to fill in the areas of loss due to the aging process!

Preparation

To minimize the chance of bruising, avoid all blood thinners such as fish oils, NSAIDS (Aspirin, Ibuprofen, Aleve, etc), ginkgo biloba, ginseng, flax oil, cod liver oil, and alcohol. You may wish to start oral Arnica tablets up to 4 days prior to your filler treatment to minimize the chance of bruising.

Dermal Fillers v. Botox

Facial fillers are also known as dermal fillers. They are intended to cause hydration and filling of the skin and hollows. Botox on the other hand is a protein that stops the communication between the nerve and the muscle, causing relaxation of wrinkles caused by muscle movement. Both are injected. Dermal fillers and Botox can be paired together to treat both the loss of volume as well as deep dynamic wrinkles to create very natural results.

Do not stare in the mirror looking for asymmetry until all the swelling as subsided! Products that may irritate the skin such as Tretinoin (Retin-A), Retinol, Retinoids, Glycolic Acid, or any “Anti-Aging” products may be best avoided. Avoid dental work for 1-2 months after. Be careful of any dirty make up or products coming in contact with your skin.

Results

All fillers have an instant filling effect seen immedinately. Sculptra however does lose its filling effect because it is more of a biostimulator. Its purpose is to stimulate your own collagen growth a few months after the treatment. Radiesse results last 1-2 years and stimulate your own collagen to create rejuvenation. It may take up to 2 weeks following injection to see the end results and swelling can occur.

Fillers are made of different products, ranging from hyaluronic, to calcium, and PLLA which are considered more natural fillers. Because they are made of naturally occurring products, they get metabolized by your skin. Synthetic materials such as Silicone last indefinitely in the skin because there are no enzymes to digest them. The thicker hyaluronic acid fillers such as Restylane Lyft and Voluma last around 1 year whereas lighter fillers such as Belotero may last around 6 months. Radiesse can last 1-2 years and stimulate your own collagen to create rejuvenation. PLLA filler (Sculptra) is actually quick to dissolve and its purpose is to stimulate the growth of your own collagen which can last years as well.
In our office, Dr. Kormeili only uses natural product fillers that she feels comfortable having injected in her own skin and family members. We do not offer synthetic fillers.

Do dermal fillers work?

It depends on the type of filler and what it is made of!
Dermal fillers revitalize and add volume to the skin, instantly diminishing the appearance of lines and hollows. Dermal filler injections can give a natural looking result if injected correctly and not overdone! Certain fillers such as Sculptra and Radiesse cause the stimulation of your own collagen in addition to the filling effect.

Are fillers safe?

Not all fillers are safe! Synthetic fillers can cause permanent defects. If injected correctly fillers can be very safe with temporary side effects such as bruising, redness, swelling, pain, and itching. If injected accidently in a blood vessel they can stop the perfusion of blood causing necrosis or skin death in the area. This is an emergency and you need to contact the office immediately for reversal treatment. Treatments with fillers under the eyes has resulted in blindness and most of the time that has been with the injection of fat. This is very rare but noteworthy as it is very important to use the correct doctors for filler injections. As a last word of caution, there are “imitation fillers” that are packaged to mimic FDA approved fillers in the US but are actually made outside the US without quality assurance. The use of these is illegal but happens still as the cost is so much lower. Never choose your filler treatment based on price for it may be the most expensive mistake of your life.

Which filler is best for nasolabial folds?

Deep nasolabial folds are generally the “sagging effect” from loss of volume in the cheek bones and jawline. If you only treat the folds you may create a heavy and “fake looking face.” Most fillers are safe for nasolabial folds and products such as Voluma or Restylane produce a great filling effect.

How long does it take for fillers to settle?

Fillers cause deep hydration and water absorption within the skin. It may take up to 2 weeks to get a new equilibrium and for the fillers to settle. If you’re unhappy with your results after this time, make a follow-up appointment for adjustments. Hyaluronic acid fillers are naturally broken down by the body. They can also be dissolved immediately upon request using hyaluronidase enzyme. It is also a great time to evaluate the area for any further need for additional fillers. It is best to start small and add slowly to minimize massive swelling and downtime.

What to expect after dermal fillers?

Slight bruising, mild tenderness, or swelling are possible. It takes a few weeks for the fillers to integrate fully into your skin and blend perfectly. During this time you may feel a bit asymmetric. True asymmetry can always be corrected after the initial swelling resolves.

Not all fillers are created equally. Most of the problems with fillers in the long term is with the synthetic ones such as Silocone because they can cause granulomas, bumps or deformity. Natural fillers made of hyaluronic acid (Restylane, Juvederm, Belotero), calcium (Radiesse) or PLLA (Sculptra) are rather safe in decades of use in the US and worldwide.
