About Aging Hands
As we age, several factors contribute to skin thinning and changes in fat distribution in the body. While younger people tend to have fat deposits spread more evenly throughout the body, older people begin to accumulate subcutaneous fat- fat right beneath the skin- in the midsection as other areas lose their volume and plump appearance. Visceral fat that builds up around the organs also increases. (1)
The aging process is also characterized by a decrease in basal keratinocytes, cells that reside just under the epidermis, the first layer of skin. These cells supply keratin, a tough protein that protects and strengthens this outer layer. (2) As the epidermis gets weaker, veins, arteries, tendons, and hand bones become more prominent. In general, candidates should have visible signs of aging on the back of their hands due to loss of fat, sun damage, or wrinkles. Patients looking to receive dermal fillers, IPL, Fraxel laser, or a chemical peel shouldn’t have any conditions related to healing. Those considering Fraxel laser treatment should have pale to light olive skin (Fitzpatrick type I through III) since people with darker skin are at risk for hyperpigmentation with this particular laser. For IPL treatments, patients with darker skin will have to apply a specialized cream to make the procedure safer.

Radiesse Injections
While dermal fillers are usually used in the face, one in particular is capable of providing sufficient volume to the hands-Radiesse. This filler is made of calcium hydroxyapatite, a naturally occurring mineral supported by carboxymethylcellulose gel which comes from the cellulose in plants. It not only volumizes creases and provides youthful smoothness, but it has been shown to increase collagen production over the course of a 9-month period, making it an effective long-term treatment that doesn’t just conceal aging. Chemical Peels
A chemical peel utilizes various compounds to exfoliate the skin at various depths- causing new, glowing skin cell regeneration. This can be a great opinion for patients with superficial scars, age spots, fine lines, and uneven skin tone. Chemical peels are quick, non-invasive procedures that may cause mild discomfort afterward. Dr. Kormeili offers the VI peel, Beta Lift peel, Jessner peel, and TCA peel; she can help you find the perfect peel that will meet your cosmetic needs and suit your skin type and/or sensitivity level.
Intense Pulsed Light (IPL)
IPL is a type of light therapy designed to reduce the appearance of redness, broken blood vessels, sunspots, and freckles, making them the perfect treatment for hands that have experienced the full effects of the sun. IPL devices are not like lasers that emit pulses of a single wavelength of light, but they emit a broad spectrum of light between 400 and 1400 nm. This treatment method doesn’t harm the epidermis of the skin, leading to little or no downtime at all.
Fraxel Laser
The Fraxel laser is a skin resurfacing treatment that utilizes fractional light that creates controlled micro injuries in some of the treatment area. This allows the skin to heal faster while still provoking the production of collagen and other connective proteins.
Recovery
It is crucial to arrange time off from work and social commitments for certain hand rejuvenation procedures. Ensuring adequate rest and avoiding strenuous activities will promote optimal results and minimize complications. Here’s the amount of recovery time you’ll need for each type of treatment:
- Radiesse injections require no downtime, but you should try to avoid strenuous exercise and sunlight. In addition, you can use an ice pack to bring down any swelling, bruising, or redness. You should also avoid lying face-down to prevent further swelling. Within a few days, all mild symptoms will have subsided.
- The downtime of a chemical peel will depend on the depth of exfoliation of the specific peel. Usually, superficial and medium peels don’t cause much irritation and only require a few days of downtime, whereas deep peels can result in up to 2 weeks of swelling and peeling. Medium peels, like TCA peels, are most often recommended for hand rejuvenation. Alternatively, you can do multiple light peels to slowly get the results you want, but this will require more treatments. When your skin begins to peel, avoid picking it off and apply a gentle moisturizer to get your skin as healthy as possible. You should completely avoid the sun until all swelling and peeling has stopped. You may receive an anti-itch cream and an antibiotic to help you heal faster. If a medium peel is used, you will have to avoid the sun for at least 2 weeks.
- Since IPL doesn’t damage the outermost layer of skin, there is minimal damage to the skin overall. For the first 2 days, you should stay out of the sun and skip your usual exercise routine. For the first week, remember to avoid picking at your face and only use gentle cleansers.
- For a Fraxel laser, you should expect to see swelling for 2 to 3 days. You should avoid sun exposure for at least 1 week and continue to wear sunscreen with an SPF of 30 or more afterward. Dr. Kormeili will provide you with a skincare kit to promote a speedy recovery.
